NOTE—In preparing for your appointment for your COVID-19 vaccine this week, please be prepared and bring, as proof of eligibility:
• Your valid state ID showing your name and date of birth, to verify you are age 65 or older (1B ages 65 and older group); OR
• Your USC ID card (for 1A, health care workers vaccination group)
You will be required to present these credentials in order to receive your vaccine.
If you are not in either of these eligible groups, PLEASE CANCEL your appointment in your Calvax link as soon as possible, as LA County orders do not permit vaccination appointments for other categories at this time. Cancelation will allow appointments for eligible seniors and 1A workers to continue without delay.
