Please be reminded that all “on site” USC faculty and staff have a flu vaccination fall deadline of November 1, 2021.
Please review instructions for uploading documentation to MySHR (the secure health records portal).
If you are a university employee and unable to obtain your 2021-2022 flu immunization documentation, you may submit a flu attestation form for the 2020-2021 season.
Individuals will see the uploaded document in MySHR as “in review.” This does not affect the record of completion.
Please note: Clinical employees, including faculty and staff of Keck Medicine of USC, may receive separate instructions and policy information specific to the health care environment.
